Each type of physical has a different purpose. A school form confirms general health, a sports form looks for anything that could make activity unsafe, and an employer form checks fitness for a specific job. We handle all three, and we read the form first.

  • School Physicals Exams for school enrollment and yearly requirements, completed on the form your school provides.
  • Sports Physicals Pre-participation exams for sports and athletics, following the form your league or school uses.
  • Employment Physicals Pre-employment and fitness-for-duty exams completed on your employer's form, with clear results for you.

Have questions about scheduling or what to expect? Here are answers to common inquiries to help you feel fully prepared.

Bring the specific form from your school, league, or employer, with any sections meant for you or a parent already filled in and signed. Also bring a photo ID, your insurance card, a list of medications and allergies, and glasses or contacts if you wear them. If you have a history of asthma, heart problems, or a past injury, bring related records so the provider can review them.

Yes. A sports physical is focused on whether it is safe for you to participate in a particular activity. The provider pays close attention to the heart, lungs, joints, and any history of concussion, fainting, or injury, and completes the pre-participation form your league requires. It does not replace a full annual physical, which looks at your overall health and preventive care needs.

In most cases the form is completed and signed at the end of the visit, as long as nothing in the exam needs further evaluation. If the provider finds something that should be checked first, such as a heart murmur or very high blood pressure, the form may be held until that is addressed. We explain why and what the next step is before you leave.

We perform physicals for patients 18 years of age and older. Call our office before booking if you are unsure whether we can complete the form for a particular age or organization, and we will let you know.

It depends on the plan and the reason for the exam. Some plans cover a yearly preventive visit that can double as a school or sports physical, while employment physicals are often paid by the employer or the patient directly. Call our office with your plan details and the type of form you need, and we will tell you what to expect before your visit.

Plan for more time than a routine sick visit. A thorough physical includes your history, the exam itself, lab draws, and a conversation about results and next steps, so we would rather not rush it. Arriving a few minutes early with your paperwork and medication list helps the visit run smoothly. Our front desk can give you a closer estimate when you book.
